The word "transition," as used in the medical community, refers to the process of preparing patients to move from the pediatric medical system into the adult system. The word "transfer" refers to the specific point in time that young adults move into the adult medical system.
Research shows that an ineffective transition process can negatively impact life- expectancy for young adults with CF. The transition process ideally starts at diagnosis. It's imperative that parents make sure their child is meeting transition goals whether or not their CF clinic has a formal transition program in place.
Transition is not just about following a checklist; it’s about empowering parents and patients with the tools, skills, and training to prepare a young adult for complete independence- including the transfer into the adult system. Our goal is to provide you with the transition tools and checklists AND provide you with the parenting education that is so critical to a successful transition and transfer.
