Here are some good transition plan resources from CF clinics around the country. As you'll soon see, CF transition plans are pretty similar. I have tried not to make these links redundant, instead focusing on the somewhat unique and unusual. Please let me know if your CF clinic uses something really different and I'll add it. Lisa

University of Missouri: The University of Missouri Health System CF Center has an excellent Developmental Education plan for use with CF Parents and Patients (created by Kecia Nelson, MSW, LCSW). 

Lucile Packard Children's CF Center at Stanford: Powerpoint show on transition by Mary Helmers, RN, BSN. Includes checklists. NOTE: This file takes a minute to load; thanks for your patience.

Connecticut Children's Medical Center Pediatric CF Clinic: Checklists by age-group for parents and children to help identify areas of strength and weakness around CF medical knowlege and shifting of medical tasks. Good parenting tips at the end.
