LINKS: CF Transition Plans from Around the US

Here are some good transition plan resources from CF clinics around the country. As you'll soon see, CF transition plans are pretty similar. I have tried not to make these links redundant, instead focusing on the somewhat unique and unusual. Please let me know if your CF clinic uses something really different and I'll add it. Lisa

University of Missouri: The University of Missouri Health System CF Center has an excellent Developmental Education plan for use with CF Parents and Patients (created by Kecia Nelson, MSW, LCSW).

Connecticut Children's Medical Center Pediatric CF Clinic: Checklists by age-group for parents and children to help identify areas of strength and weakness around CF medical knowlege and shifting of medical tasks. Good parenting tips at the end.

Washington State Dept of Health: This site includes nice color transition resources. I like the simplicity of these resources including the checklists.

The Institute for Child Health Policy at the University of Florida: This is an amazing webpage with many excellent resources including videos, beautifully done full color transition booklets and more. Take the time to visit this one...
